Site safety records for the PSCS and main contractor – inductions, sign-in, permits, inspections, toolbox talks, RAMS and actions
Induct every person with their Safe Pass and CSCS details, log who is on site each day, issue and close permits, tick the weekly inspection and record toolbox talks, RAMS and actions – and see on one dashboard what needs dealing with today.
On most small and medium sites the PSCS paperwork lives in a ring binder, a sign-in book at the gate and a few loose spreadsheets. When the client, the PSDP or an HSA inspector asks who was on site, whether a permit was closed out or when the roofers' RAMS were accepted, it takes time to find – and an expired Safe Pass or an open action is easy to miss.
This workbook keeps the construction-stage site records together. Each person is inducted once, with their Safe Pass and CSCS card expiry; the daily sign-in picks up their company and flags anyone not inducted or with an expired card. Permits show 'valid until' and their status to the minute, the weekly inspection gives a score and a list of ✗ items, toolbox talks record who attended, and RAMS and actions show what to chase. It sits alongside your construction stage Safety and Health Plan – it does not replace it.
Key figures, quick links and a step-by-step guide.
Project and duty holders (client, PSDP, PSCS), the HSA notices – AF1 sent by the client and AF2 by the PSCS, flagged if they do not match – key dates and warning windows, permit types with hours valid, the companies on site with their status, and your toolbox talk topics with how often to repeat them.
Everyone inducted (up to 300): company, role, Safe Pass and CSCS card with expiry dates, signed, first aider – status, days to expiry, last day on site and toolbox talks attended.
Daily sign-in / sign-out (3,000 lines): company from INDUCTIONS, hours on site, and a check that flags anyone not inducted or without a valid card on that day.
Permits to work (200): type, location, work, company, holder, issued and 'valid until', close-out and fire watch – status Active, Expires soon, Expired – close out, Closed or Cancelled.
Weekly site inspection: 58 checks in 13 areas (scaffolding, excavations, plant, housekeeping, PPE, signage and more) × 52 weeks with ✓ / ✗ / N/A, a score and ✗ count per week.
Toolbox talk register with date, topic, who gave it and to whom, plus an attendance log (one line per person per talk).
Risk assessments and method statements received from each contractor: received, reviewed, decision, briefed to workers – status and days waiting.
Inspection findings, unsafe acts and conditions, near misses and accidents, each with an action, owner and due date – plus injury details and an 'over 3 days' reminder.
On site today, active permits, open actions, lost-time-free days, four charts and a 'what needs attention' list with links to each register.

The Project Supervisor Construction Stage (PSCS) is the duty holder appointed by the client under the Safety, Health and Welfare at Work (Construction) Regulations 2013 to coordinate safety and health during the construction stage. The PSCS prepares and develops the construction stage Safety and Health Plan, coordinates the contractors on site and organises things like site rules, inductions and cooperation between trades. On many projects the main contractor is appointed as PSCS.
The HSA's BeSMART.ie service has a free construction stage Safety and Health Plan template for the PSCS. This workbook is not a plan template – it is the set of site registers that sits alongside the plan: inductions, daily sign-in, permits to work, weekly inspections, toolbox talks, RAMS and actions.
Under Regulation 6 of the 2013 Construction Regulations the client appoints a PSDP and a PSCS in writing where more than one contractor is involved, where the work involves a particular risk (such as those in Schedule 1), or where the project must be notified to the HSA (over 30 working days or 500 person-days). The PSCS is appointed before construction work starts. This workbook records the appointments and notices – it does not decide whether they are needed.
PSDP is the Project Supervisor Design Process, who coordinates safety in the design and prepares the preliminary Safety and Health Plan and the Safety File. PSCS is the Project Supervisor Construction Stage, who coordinates safety on site while the work is carried out. Both are recorded in SETTINGS together with the client and the HSA notices (AF1 from the client, AF2 from the PSCS).
It checks the expiry dates you enter. INDUCTIONS flags expired, missing or soon-to-expire Safe Pass and CSCS cards, and each daily sign-in is checked against the card dates on that day, so anyone not inducted or with an expired card shows up in red. It does not verify cards with SOLAS – always see the card itself.
No. The Safety File is compiled by the PSDP and handed to the client at the end, with information needed for future maintenance and work on the building. This workbook holds the construction-stage site records kept by the PSCS – some of that information can help when the Safety File is put together.

A PSCS template for the day-to-day site records: a site induction register with Safe Pass and CSCS expiry dates, a daily sign-in sheet, a permit to work template (including a hot work permit, confined space entry and excavation permits) and a weekly site inspection checklist covering scaffolding, excavations, plant, housekeeping, PPE and signage.
It also works as a toolbox talk register with attendance, a RAMS register for the method statements your subcontractors send in, and a log of near misses, accidents and corrective actions – the site safety records for small and medium construction sites in Ireland (not the PSDP's Safety File), with a dashboard showing what needs attention.
